Free sports physicals at Rural Retreat High School May 17, 2012 beginning at 2:00pm. 
Students may pick up the forms from the office. Do not fold or tear forms. 
Complete parts I, II, IV and V on the physical form with signatures 
and return to Mr. Hughes by May 14th.  Incomplete forms 
will be returned for completion.
